Primary Accountability:
The Care Coordinator ensures efficiency and effectiveness in the patient experience and patient navigation. They will work directly with network patients to focus on coordination of care including routine communication directly with patients, collaborating with care teams at the health centers, facilitating care transitions, connecting patients with referrals/resources, and providing linkages to other community-based or social services. This includes quality improvement and assurance reviews involving patient navigation, management and support by coordinating with other healthcare members to provide quality care for every patient.

Major Duties:

  • Conducts review assessments, needs assessments, treatment planning and reassessment services.
  • Designs and delivers individualized and group coaching at practices by working with providers to identify site-specific needs and opportunities, prioritizing areas of focus, and identifying available and needed resources.
  • Designs and delivers individualized and group coaching at practices for workflow implementation and optimization to improve and/or enhance disease management and treatment that is patient centered.
  • Reviews patient cases with providers and provides advice, direction, and support.
  • Organizes and leads training sessions, on process recommendation and updates.
  • Meets with providers, patients, and primary care physicians to review and update care plans.
  • Participates in the Missouri Quality Improvement Network and supports member Federally Qualified Health Centers (“FQHC”) in quality improvement activities, including quality improvement and performance improvement coaching.
  • Works collaboratively with other practice coach staff for reporting, training, and program coordination.
  • Facilitates specific projects among member and partner organizations, including State agencies and local/regional organizations as assigned.
  • Preforms additional duties as assigned

Qualifications:

  • Bachelor’s degree in nursing, public health, health care administration, or other related health care field.
  • LMS/LCSW/LPN or RN certification or licenses, experienced Medical Assistant, Community Health Worker or equivalent considered
  • Two or more years’ experience in an outpatient clinic setting or in an area of quality improvement, clinic/practice management, and/or clinical area highly preferred.
  • Experience with Missouri based Federally Qualified Health Centers or outpatient primary care practice highly preferred. Training and/or clinical area highly preferred.
  • Proficiency in all Microsoft Office products
  • Strong interpersonal communication skills with the ability to effectively communicate, both written and verbal, in a professional manner that supports the ability to establish and maintain effective working relationships across all levels of MPCA and external stakeholders.
  • Strong organizational skills, the ability to prioritize, and the ability to coordinate multiple projects with minimal supervision.
  • Ability to work in team settings to show flexibility and versatility.
  • Ability to learn additional software applications.
  • Ability to self-motivate and thrive in a complex and rapidly changing environment.
  • Ability to work with strict confidentiality requirements, as well as utilize good judgment in the preparation and distribution of confidential information.

Physical Demands:

  • Ability to sit, stand, walk, reach and use hands and fingers for extended periods of time.
  • Performing physical activities which require considerable use of your arms and legs for moving your entire body, such as climbing, lifting, balancing, walking, bending, kneeling, crawling, stooping, and handing of materials.
  • Ability to read computer screens, mail, e-mail and talk on the phone.
  • Ability to unpack and move supplies up to thirty (30) pounds.
  • Ability to drive an automobile and possess a valid driver’s license.

Work Environment:

  • Professional and deadline-oriented environment in an office setting.
  • Interaction with internal and external customers.
  • Job duties will be performed either from a remote office location or onsite, at the Jefferson City, MO location.

Travel:

  • Travel will be expected in and outside of Missouri.
